Professional Certificate Programme in Musculoskeletal Pain Management in Primary Care

Notes for Applicants
1. Submission of Application Form
Please fill in the online application form and submit ALL required documents to Course Administrator of “Professional Certificate Programme in Musculoskeletal Pain Management in Primary Care”.  Insufficient supporting documents will delay the application process. Quotas are available on a first-come-first-served basis. Programme Committee reserves the right to make any final decision.
2. Documents to be Submitted
Supporting documents should be sent to pain@cuhk.edu.hk via email (document 1 and 2) with an email subject “MSK 2024 Application – (Your Full Name)”.
(1) (By Email) Photocopy of identity card 
(2) (By Email) Copies of qualification certificates (including Professional Qualifications, Highest Academic Qualifications
(3) (By Post) An application fee of HK$300 (A crossed personal cheque made payable to: “The Chinese University of Hong Kong”).
Please write your name, contact number and “Application fee for MSK cert” at the back of the cheque.
Original documents must NOT be sent.
3. Postal Address

4. Application Deadline
Application form altogether with the supporting documents should be sent to the above postal address on or before 4 October 2024.
5. Selection Process
All application will be reviewed by the Programme Committee. Shortlisted candidates might be invited for an interview.
Successful applicants will receive a confirmation. Programme fee should be settled within 2 weeks after confirmation.
6. Tuition Fee Payment Methods
Tuition fee (HKD 36,000) should be paid by a crossed cheque payable to “The Chinese University of Hong Kong” within 2 weeks after confirmation.
All fees are NON-REFUNDABLE and NON-TRANSFERABLE.
